Vouchers and gifted plans charge nothing, so they earn nothing. A refunded payment takes its commission back. Referring yourself, or accounts you control, is refused, and suspected abuse freezes commission until a person reviews it.
Yes, and a change applies to future payments only: every commission keeps the rate it was earned at. The current numbers are always the ones on this page and on /pricing.
